Living Healthy Steuben!
Living Healthy Steuben! is a group of local individuals who meet once a week, for six weeks, in community settings such as senior centers, churches, libraries and hospitals. People with different chronic health problems attend together along with caregivers of those living with a chronic condition. The group is facilitated by two trained community leaders. It is the process in which the program is taught that makes it effective. Groups are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.

The locally known Living Healthy Steuben! is the Stanford University Chronic Disease Self-Management Program.
The Steuben Rural Health Network receives major funding from grants through the New York State Department of Health, Charles D. Cook Office for Rural Health and the Community Health Foundation of Western & Central New York to be able to implement this program in Steuben County.
